My name is Kassandra “Nurse K” Fletcher and I am the full-time Certified School Nurse. I am so happy to be your school nurse and look forward to partnering with you to ensure a healthy and successful school year for all our students! Below you will find information and direct links to most necessary forms on this page that you may need to complete. We kindly ask that you return any completed forms to the school nurse.

1. Annual Student Emergency and Medical Information Form (S-865):
a. This form must be filled out every year by parents or guardians. It gathers vital health history for your child, allowing the school nurse to provide necessary care, including administering Acetaminophen, Ibuprofen, or prescribed medications like Albuterol or EpiPens. Please note that our nurses cannot administer these medications without your permission.
b. This form can also be completed online on the John Barry website under the parent tab. I have also included the web address below.
2. Immunization Requirements:
a. We require specific immunizations or an exemption for all grades (K–12) to maintain a healthy school environment. Please send in immunization records. If your child needs care but you lack health insurance, please visit https://www.phila.gov/media/20240912150038/Find-A-Clinic-FQHC-Handout-9-12-2024-EN.pdf or visit www.phila.gov/health for free and low-cost options.
b. Exclusions will begin on October 6th, 2025. Please monitor your student’s backpack for requested missing information
i. Dtap: 5 doses
ii. Polio: 4 doses
iii. MMR:2 doses
iv. Hep B: 3 doses
v. Varicella: 2 doses
vi. 7th grade: 1 dose of Tdap and MCV4
vii. 12th grade: 2 doses of MCV4
3. Medications at school (MED-1, MED -4, S865):
a. If your child must have medication of any type during school hours, INCLUDING over-the-counter medications, you must adhere to the following regulations, under state law:
i. Required forms: MED-1, MED-4, & S865
ii. Original container (pharmacy label, name, DOB, order information)
iii. Delivered by Adult
b. The following medications are available to be administered by the school nurse if the student has a current and complete S-865 Form on file.
i. Acetaminophen (Tylenol)
ii. Ibuprofen (Motrin, Advil)
iii. Hydrocortisone cream 1% (Anti-itch)
4. Health Screenings:
a. In accordance with the Pennsylvania Public School Code, our school nurse will conduct specific health screenings for students in certain grades. These screenings are designed to ensure your child’s health and well-being during their school years. These screenings do not replace regular checkups with a pediatrician.
i. Growth (height and weight) for K-12
ii. Vision screening for K-12
iii. Hearing screening for grades K, 1, 2, 3, 7, and 11
iv. Scoliosis for grades 6 and 7
5. Student Illness & Attendance:
a. It’s crucial that students attend school when healthy.
a. If your child is not feeling well and you are uncertain about sending them to school, it is best to keep them home and observe them for worsening symptoms.
b. When you make the decision to keep your child home, notify the school of their absence.
c. Keep your child home until their symptoms are not present and they have no diarrhea, vomiting or fever (without taking medications) for at least 24 hours.
d. If your child’s symptoms are not improving or are getting worse, visit the doctor.
6. Physical (MEH-1) and Dental Exams (MEH155):
a. Pennsylvania law requires a physical exam and dental exam upon original entry, in grades K, 6, and 11. These should be completed by your child’s primary care provider and returned to the school nurse.
7. Accessing Medical and Dental Records (Release of information)
a. If you are having difficulty obtaining your child’s medical or dental records, or if you do not have access to them, we may be able to assist with the completion of a Release of Information form. This form allows us to contact your child’s healthcare provider directly to request the necessary information. Please reach out directly if you are in need of this form
